Monandnock Region Partners in Health program
Update

Partners in Health is a community based, Family Support program designed to address the needs of families who have children with chronic conditions.
Directed by family concerns and maintaining a family-centered approach, we work within a family/child's community to facilitate and enhance the care and services needed by the family.
Partners in Health works with schools, medical providers, social services, and other community organizations to create a setting that will enhance a family's quality of life.
Who we serve:
Families with children from birth to age 21 with a chronic health condition:
*certified by a physician
*expected to last 6 months of longer
*condition affects daily life
Examples of diagnosis:
Autoimmune deficiencies, Diabetes, Respitory diseases, Heart conditions, Cancer/Leukemia, Genetic disorders, Intestinal diseases, Muscular disorders, Bone diseases, Neurological diseases, and rare diseases.
Areas of Assistance:
Services are tailored to each specific family. Examples: Referral to related services, Referral to grant/loan programs, Recreation, Medical/educational equipment, Respite.
Family Council:
A group of parents who have children with chronic health conditions that meets regularly to discuss related issues and share personal experiences. The Council assists the Coordinator in developing projects and/or activities that will benefit families in the region who have children with chronic health conditions.
Support Group:
For Parents of children with special needs and chronic health conditions. Facilitated by a LMFT. Meets the first Monday of each month at MDS, 121 Railroad St. Keene from 7:00 pm to 8:30 pm.
Parent Discussion Group for Parents of children with Tourettes Syndrome. Meets the 2nd Tuesday of each month at MDS, 121 Railroad Street from 6:30 to 8:00 pm.
