Partners in Health is a program designed to assist families with children with special medical needs. It is a community based, family support program, directed by the needs of individual families. The goals of Partners in Health are to provide families with support, help families with the challenges they face, provide information, and be an outlet for stress. Partners in Health serves families with children, ages birth through 21, with a chronic health condition (certified by a physician, expected to last 6 or more months, affects daily life). Some examples of diagnoses of individuals served by the program include: cancer, diabetes, genetic disorders, epilepsy, spina bifida, cerebral palsy, heart conditions, pulmonary disorders, blood disorders, bone diseases, brain and neurological diseases, and more. Services are determined by the needs of the family, some ways that Partners in Health have helped include:
navigating medicaid and HMO's, advocacy, parent and child education, respite, recreation, locating information, referrals to grant programs, community awareness, etc.The NH Partners in Health program is a collaborative program, which is guided by the regional Family Council and Coordinator. The Family Council is made of parents of children with special health needs. The Family Council in the Lebanon area is also comprised of two individuals who have health conditions, both who were diagnosed as children, and are now adults. The Council assists the Coordinator with community development and planning activities that will benefit the members of their community. The council meets monthly. Anyone can make a referral - parent, relative, friend, doctor, teacher, child care provider, etc. The Partners in Health Coordinator in the Lebanon area is:
